This fall, TEAM schools should have the TEAM evaluation cycle completed by November 14th. A typical cycle takes place over a week with some extending into a second week when necessary. The goal is for each resident to be formally observed by three different evaluators. It is recommended that the resident's assigned mentor conduct the first observation followed by two additional mentors. These observations should take place on different days and in some cases with different classes/subjects. A date/time will be scheduled by the school with the clinical liaison to meet as a team to complete 1 TEAM evaluation. Mentors will attend this meeting with completed individual evaluation forms and any evidence/documentation.

RESIDENT:
___Submit summative Pre-Conference Form to all three mentors conducting evaluations at least 3 days prior to evaluations.
___Submit official Louisiana Tech lesson plan and all teaching materials to each mentor.
___Meet formally with evaluation team prior to observations.
___Meet informally after each evaluation with mentor for immediate feedback.
___Submit Post-Conference Form no later than 48 hours after evaluations.
___Meet formally with assigned mentor after TEAM evaluation to determine plan for future professional growth.
___Submit all required documentation to Louisiana Tech electronically.
MENTOR:
___Determine evaluation plan (dates/time frame) for all residents at school with mentor team.
___Review Pre-Conference forms from assigned residents to evaluate.
___Schedule a Pre-Conference formal meeting with resident prior to observation. See attached file below providing guiding questions for pre-conference.
